In addition to the duly completed application form (including postal and phone contact details) signed by the applicant with a handwritten signature, the following documents are required:
- A verified and certified copy of the original academic qualification, legalized via Apostille by the Foreign, Commonwealth & Development Office (FCDO). Verification and certification must be carried out by a UK Notary Public. IMPORTANT: The notary must explicitly confirm the verification of the qualification and the accreditation of the issuing institution, for example, using the statement: “I certify that this degree is authentic and that I, the notary, have verified it with the relevant institution,” in either English or Italian.
- A simple (non-certified) translation of the degree certificate, which can be done by the applicant or a third party. PLEASE NOTE: Only the degree certificate (the diploma) must be translated into Italian. Furthermore, the translation into Italian must retain the following information in English: the name of the university, the degree title (e.g., Bachelor of Arts, Bachelor of Science, Master of Arts, Master of Science, Doctor of Philosophy, etc.), the field of study if applicable (e.g., Business Management, International Relations, Biology, etc.), the grade if present (e.g., First Class Honours, Second Class Honours Upper Division, Merit, etc.), and, if applicable, the name of the faculty (e.g., Faculty of Arts and Humanities).
- The Diploma Supplement (DS) or HEAR (Higher Education Achievement Report) or original Transcript of Records. Alternatively, a verified and certified copy of the original Diploma Supplement (DS) or Transcript, legalized via Apostille by the Foreign, Commonwealth & Development Office (FCDO). These documents must include the following information: full name, date of birth, date of first enrollment, program duration, study mode (full-time or part-time), number of academic credits earned, and whether a year abroad or internship was included in the program (specifying the university or company involved). If the Transcript, Diploma Supplement, or HEAR does not contain the required information, please request an additional statement (letter) from the British university, which must be provided in the original.
- A verified and certified copy of the original QTS Certificate, legalized via Apostille by the Foreign, Commonwealth & Development Office (FCDO) by a UK Notary Public.
- A simple (non-certified) translation of the QTS Certificate, which can be done by the applicant or a third party.
- An original confirmation letter for the QTS issued by the Department of Education (previously National College for Teaching and Leadership – NCTL), Teaching Regulation Agency, and previously General Teaching Council.
- A verified and certified copy of the original Induction Certificate, legalized via Apostille by the Foreign, Commonwealth & Development Office (FCDO) by a UK Notary Public.
- A simple (non-certified) translation of the Induction Certificate, which can be done by the applicant or a third party.
- For UK shipments: A pre-paid, pre-addressed A4-sized special delivery envelope with the address to which you would like to receive the declaration of value, the certified translation, and the original documents. If you wish to receive the declaration of value in Italy or another foreign address, do not include any envelope. Instructions will be provided via email during the processing of your application on how to arrange a courier pickup at the Consulate at your own expense.
For the issuance of Declarations of Value related to PGCE, including QTS and INDUCTION, the following consular fees will apply: art.66n (issuance of the Declaration of Value) + art.72a + art.72a + art.72a for the three translation conformities required (academic qualification + QTS + INDUCTION).
It is confirmed that all original documents (Official Transcript/Diploma Supplement/Original Letters) will be returned, and that the Declaration of Value will include only the verified, certified, and legalized copy of the original qualification, the certified translation of the qualification, and the verified, certified, and legalized copies of the QTS and Induction Certificates, along with their certified translations, all bound together in a single document.
